One of the more enjoyable shifts I spent in nursing was while employed by a temporary agency working in a Seattle hospital. I had an order to feed solid food to a patient who had not been eating prior to that and found that he would choke on the food. So I charted this and didn't feed him. The doctor had a fit. I charted his comments to me which included the one about any liablity had I proceeded to be on his head, not on mine. That was after I rephrased what he said and asked him if I would be exonerated completely if the person choked and died as result of the food I gave him. His ego was such that he stupidly said that would be true. I documented all of this and then called for the Supervisor of Nurses on that shift. She came up, read the chart, and we had a nice little chat with several of the other staff on that unit hanging on every word said. The upshot is I didn't feed the man. And, I would have been very liable if that man had choked on the food and died or had any other problems. ANY CHANGES IN MEDICATION orders come from the pharmacy department of any large hospital I had an order for around 10 times as much Heparin as my patient could tolerate on another occassion. I simply sent the order down to the pharmacy department with a request for the pharmacist on duty to contact the doctor in question and get it straightened out. I would not have given such a dose as my own license would have been in jeapordy to say nothing of how I would have felt if I had harmed the person. That is how nurses handle lousy orders on charts, Alan.
