These are -> the same people that would yell when we brought COPD patients in on -> high flow O2, because it would make them stop breathing. Another -> myth with amazing resilience. Ya know, I finally saw a case where there might have been a 50-50 percent chance that the pt's resp. arrest was caused by overoxygenation. Severe COPDer, physician at E.R., also our med control doc, said that it was that or it was just time for the pt. to die. I would have figured the latter. Pt. hadn't been on NRB O2 but for about 5 minutes and resp. arrested as the crew brought him out of the back of the box (I was standing nearby and made them aware that they're pt. "doesn't look good"). Anyways, that's the only one where there was a chance that it was secondary to the O2 that the doctor had been involved in. Point-of-care testing in the hospital is increasing in popularity, and it certainly has potential application in the field as well. The portable ABG analyzers, such as the Irma and I-Stat, are hand held, have a disposable analysis cartrige, and are fairly fast and accurate. Right now, their main use is for faster response time in emergency situations in the hospital - for example they can be taken to a code blue. I think they have potential value in the home care setting, also. But I'm not sure how useful they would be on an ambulance... my inclination is that the results would not alter your interventions that significantly during the pre-hospital time period. My Fire Dept has a really cool Medical Director who lets us do pretty much what we want. Meaning our protocols are extremely liberal. To the point he had to fight with the state to accept them. We do what needs done then call the hospital. This whole to do is predicated on Medical politics, and what doc happens to think what about whatever. Docs don't like their parade rained on, they are extremely jealous of their perogatives and as such we as field people have little consistancy in how we are allowed to care for our patients. However getting the Nat'l Registry or DOT involved would be the anathema of common sense and effectively kill competent pt care as the degree of anal retention associated with these groups is astronomical!!!
